Controller and contact

Urban Sky Web Ltd is the controller of personal information handled through MyPHU.com. It is registered in England and Wales under company number 17421062. Its registered office is 14/2E Docklands Business Centre, 10–16 Tiller Road, London, E14 8PX. Privacy questions and rights requests can be sent by post to that address.

Information processed and why

If you choose a country, MyPHU stores that choice in an essential session cookie so it can display the correct emergency number and service links. The legal basis is Urban Sky Web Ltd's legitimate interest in providing a useful, country-aware and secure service.

When you submit the self-check, its health-related answers are processed briefly in server memory only to calculate and return the result on screen. They are not written to the MyPHU database. MyPHU relies on your consent under Article 6(1)(a) UK GDPR and your explicit consent under Article 9(2)(a) for this special-category health information. The checker is optional and emergency information remains available without it.

The hosting provider may process technical logs such as IP address, browser type, requested page, time and security events. These are used under legitimate interests to deliver, secure and troubleshoot the site.

Analytics cookies

Google Analytics remains off unless you actively accept analytics cookies. If accepted, Google may receive online identifiers, device and browser information, approximate location, visited pages and interactions. MyPHU disables advertising storage, signals and personalisation. You can reject analytics without losing access, or reopen Cookie settings in the footer and change your choice at any time.

Recipients and transfers

Information may be processed by providers supporting hosting and security, and by Google only when analytics consent is given. When you choose an external official health or support link, the destination processes information under its own privacy notice.

Some providers may process information outside the United Kingdom. Where required, Urban Sky Web Ltd relies on an applicable adequacy regulation or approved contractual safeguards and supplementary measures. You may ask for information about the safeguard used.

Retention

Self-check answers are discarded after the response is produced and are not retained in the database. The country preference lasts for up to one year. The analytics choice remains in browser storage until it is changed or cleared. Google Analytics data and hosting or security logs follow the relevant provider's configured retention schedule; those settings and providers are reviewed periodically.

Your rights

Depending on the circumstances, you may request access, correction, deletion, restriction or portability of your personal information. You may object to processing based on legitimate interests. You may withdraw analytics or self-check consent at any time, without affecting processing that was lawful before withdrawal.

MyPHU does not use stored personal information for solely automated decisions with legal or similarly significant effects. The self-check applies fixed signposting rules only to show an immediate, non-diagnostic result.

Complaints

Please contact Urban Sky Web Ltd first so it can investigate. You also have the right to complain to the Information Commissioner's Office.

Medical information

Do not send personal medical details to MyPHU by post or through unrelated channels. MyPHU does not currently provide accounts, messaging or medical-record storage.
